Duplicate menu items

I have two top-level menu items for ReSharper -- one is dimmed. I realize I
must have uninstalled ReSharper incorrectly at some point, but is there a
way to get rid of the dimmed item? I was hoping that reinstalling would use
the dimmed item, but instead another item was added. And I can't figure out
where such information is stored!

Hello Mark,

you can edit the menu structure using the VS dialog Tools|Customize.

Hello Mark,

I think it's not you, only very few users are aware of this VS functionality.
Actually we
also discovered it only long time after we started to use VS 2005.
