What is the correct pattern for Actions and ActionGroups?

What is the recommended pattern for implementing Actions and ActionGroups? Is it still necessary to define the Actions in an xml resource file? The only documentation and examples I have seen appears to be either deprecated or incomplete. Is there an API reference for Resharper SDK that I have missed?
