The contextual items work for me in 778. Their enabled state is a little more aggressive than it used to be. However, I like the new functionality of only enabling things that are appropriate for the item you right-clicked on.

That's strange … We didn't expect any such changes in recent builds. Could you please provide a couple repro cases? That could as well be a degradation.

Both items work now.
I switched to solution explorer and back and the cursor came back.
And when the cursor came back, the refactoring items came back as well.
