ReSharper 4.1 failed to load or caused an exception when VS2008 starts

I get the dialog text below from the VS2008 add-in manager on start up.  Even after VS starts I can't use the add-in manager to run resharper without getting this same error.  The same is now happening with VS2005.

I'm using JetBrains ReSharper add-in for Microsoft Visual Studio 9.0.
ReSharper 4.1 Full Edition build 4.1.933.3 on 2008-09-02T17:45:28.
I also get the same result using version 4.1.943.5.

Any ideas on what's wrong?  What can I provide to help resolve this issue?
