Feature request 2

AFAIK this refactoring is named Introduce Field and we already have such request in the tracker. We'll definitely implement it in future.

I'd like to see the addition of the refactoring "Introduce Instance
Variable". This would work exactly like "Introduce Variable", but the
option to replace all occurences would span all methods.

Conversely, an "Inline Instance Variable" would do the opposite,
similar to "Inline Variable".
