Installing 402 on top of 351 leaves R# 2.5 available as a plugin. Should
the installer remove 2.5 before installing 3.0?

Yes, it should remove 2.5. Why do you think it leaves 2.5 as plugin?

Hi Oleg - when VS started it asked me if I wanted to disable R# 2.5, and
2.5 still shows in the add-in manager.
