Remove UserSettings.xml in "C:\Documents and Settings\\Application
Data\JetBrains\ReSharper\v4.5\vs9.0". Replace parts of path as appropriate
to your OS, VS and ReSharper versions. We will add button to License dialog,

This will not do the trick. You need to clean up the registry either HKEY_CURRENT_USER\Software\JetBrains\ReSharper\v4.5\vs9.0\LicenseSettings where v4.5 should be replaced with the ReSharper version you're cleaning out, and vs9.0 is the version of VS for which ReSharper was installed
